Diclofensine (Ro 8-4650) is a new isoquinoline antidepressant. The only commercially available antidepressant containing the basic isoquinoline moiety of diclofensine is nomifensine, from which it differs markedly in its pharmacological profile. Diclofensine is a strong inhibitor of serotonin, noradrenaline and dopamine uptake and it is this polyvalent effect on the three neurotransmitter systems suspected to be involved in the affective disorders that provides the basis for its use as an antidepressant. Early exploratory, uncontrolled trials suggested the usefulness of this new drug as an antidepressant. The response was characterized by psychoenergizing, mood alleviating effects. These trials also provided the first estimates of the range of clinical dosage. Diclofensine is being developed as a potential member of the ‘new generation’ of antidepressants. This drug has major structural differences from the tricyclic antidepressants.
